Community Health Screening App

A nonprofit runs free kidney-health screenings at community health fairs. This app is how those events are run on the ground: volunteers on tablets register walk-up participants, capture consent, work through screening questionnaires and leave follow-up notes; participants on phones join events, fill in their health history and read their own results.

Consent is treated as a record, not a checkbox: a paginated bilingual flow, eight pages on a phone and three on a tablet, with the agreement and its timestamp persisted to the participant's account. Phone verification runs through Firebase with SMS autofill, and app attestation protects the API.

The phone and tablet layouts are separate screens chosen per feature by device size, with dimension scaling against each design frame, so a volunteer's tablet form and a participant's phone form each fit their device.
